Minnesota Department of Health (MDH) Cardiovascular Health Unit
Mission: Improve the cardiovascular health and reduce the burden of heart disease and stroke for all people living in Minnesota, so that all Minnesotans can lead heart-healthy lives.
Through funds from the Center's for Disease Control and Prevention, the MDH Cardiovascular Health Unit aims to support and develop new and innovative ways to address:
Heart disease
Stroke
Emphasis on health equity
Many initiatives align with the
Million Hearts Program
and include strategies aimed at increasing referra, participation, and completion of cardiac rehab.
