Award of Excellence

Each year we strive to recognize an outstanding individual who works in the field of cardiac and/or pulmonary rehab. Whoever is chosen as the recipient is then recognized at the spring conference. Please see the criteria below and let us know if you have someone to nominate for the award.

Criteria:

  • A nominee should have made significant and unique professional contribution(s), which have had an impact on cardiac and/or pulmonary rehabilitation.
  • A nominee could be recognized for expertise in the areas of clinical practice, research, leadership, professional education, or a combination of these.
  • A nominee should be a professional in the field of cardiac and/or pulmonary rehabilitation.
  • A nominee may or may not be a current member of AACVPR/MNACVPR.
Nomination Requirements:
  • A brief nomination letter explaining why the person is being nominated e-mailed to wquast1@fairview.org by March 1, 2023
  • The board will follow up to gather more information if needed

 

The winner will be announced and presented with their award at the Spring MNACVPR Conference

Innovation Award


11/1/2022 Update for 2023 Innovation Award 
Innovation and Quality Improvement Award application will be available early 2023.

MNACVPR and MDH are excited to offer Minnesota Cardiac and Pulmonary Rehabilitation programs funding to improve resources within their programs and increase patients’ involvement in their care. All current affiliate members of MNACVPR/AACVPR are encouraged to apply. The Innovation Committee and the MNACVPR Board will review all applications and select the winner(s).  Multiple awards may be issued, up to $5,500.  Programs may submit multiple applications. Winners will be recognized at the state conference on April 20th-21st, 2023.

What is different from previous years’ awards?

This year’s application has been funded by both MDH and MNACVPR. This allows for broader use of awarded grant funds. Programs may apply for ideas innovating for their own program improvement and patient engagement.
