Minnesota Department of Health (MDH) Cardiovascular Health Unit

Mission: Improve the cardiovascular health and reduce the burden of heart disease and stroke for all people living in Minnesota, so that all Minnesotans can lead heart-healthy lives.

  • Through funds from the Center's for Disease Control and Prevention, the MDH Cardiovascular Health Unit aims to support and develop new and innovative ways to address:
    • Heart disease
    • Stroke
    • Emphasis on health equity
  • Many initiatives align with the Million Hearts Program and include strategies aimed at increasing referral, participation, and completion of cardiac rehab.

MDH meets with the MNACVPR Board regularly to collaborate on patient and program outcomes. MDH also shares resources they've created for any cardiac and pulmonary rehab clinic to use.
